Findings
Mechanical seal on P-101 was leaking; the seal faces were scored.
Work performed
Replaced the mechanical seal (28 mm) and the casing gasket on P-101. Ran the pump after the repair; no leakage from the seal.
Recommendations
Gate valve V-12 (discharge side): bonnet weeping with some rust. Replace the bonnet gasket at the next PM.
